Movement analysis and special testing, clear definition of diagnosis, prognosis of time of rehab visits required, and medical billing invoice for self submission to private insurances. CliffPT is not a medicare or medicade provider. All follow Up Session are 60mins at $175/hr Travel Tolls & Parking Garages Expenses greater than $25 not included in eval or follow up visit fees.
